It can be difficult and time consuming to assist consumers moving back into the community from nursing facilities and other institutions. But don’t let that stop you from doing it! Successful transitions from institutions back to the community often require coordination of all of your CIL’s core services. IL skills are especially important to make sure consumers that move home – stay there and live happily and independently. Our presenters will explain how to apply what you already know and how to expand on your IL skills training specifically for individuals who are moving to the community. Find out how you and your staff can assist consumers to identify, learn (or re-learn) the skills that will increase the chances of a successful transition.
